Request Immunization and/or Health Records
Request Records for Current and/or Previously Enrolled Students:

Students who have been enrolled in Saint Paul Public Schools may also request copies of their immunization records for the time they were enrolled in the district.

To obtain this information, please fax requests to Student Data Services, 651-632-3757, with the following information:

  • Full name used in school
  • Date of Birth
  • Last school and year attended in Saint Paul Public Schools
  • A telephone number where you may be reached if further information is needed
  • Complete mailing address for return mail
